Question: Hi HAve fibroid, please suggest next step..

below is ultra sound report

Age / sex : 37 yrs / Female Ref By ULTRASOUND ABDOMEN and PELVIS Liver appears normal in size, shape and echopattern. No obvious focal lesions seen. No evidence of intra / extra-hepatic biliary tree dilatation noted. Portal vein appears to be of normal size. Gall bladder well distended. No definite calculi identified. No evidence of abnormal wall thickening noted. Spleen measures - 8.3 cm and appears normal in size, shape and echopattern. No focal parenchymal lesions identified. Pancreas partially obscured by bowel gas. The visualized portion appears normal in size, shape and echo pattern. No definite calcification or ductal dilatation noted. Right kidney measures - 105 x 36 mm. Left kidney measures - 97 x 45 mm. Both kidneys appear normal in size, shape and echopattern. Corticomedullary differentiation appears maintained. No evidence of calculi or hydronephrosis seen on either side. IVC, abdominal aorta and para aortic regions are obscured; No significant lymph node enlargement identified. Urinary bladder distended and appears normal. No evidence of abnormal wall thickening noted. Uterus anteverted and appears normal in size, measuring - 72 x 49 x 50 mm in size. Myometrial echoes appear mildly hetrogenous. Endometrium measures - 11.7 mm. Both ovaries are partially obscured; Right Ovary measures approximately 37 x 22 mm; Left Ovary measures approximately 44 x 18 mm; and is significantly obscured by Fibroid shadows; Both Visualized Ovaries appear within normal limits. No free fluid is seen in the pelvis.

IMPRESSION: Scan done on Voluson E8 and XXXXXXX V7. A well defined, large heterogeneously hyper echoic, fundal fibroid is seen, subserous, broad based polypoid, and measuring approximately 140 x 90 x 130 mm. Uterine, previous LSCS Incidentally detected, seen - appears within normal limits; Incidentally detected, Multiple thin walled loculated septate region in the right inguinal canal and extending into the right labia, shows slow flow within it ? Lvmphangioma . IT measures approximately 7.5 x 1.5 x 4 cms. Peritoneal collection is also seen in the right inguinal canal measuring approximately 3 x 0.5 x 1 cms.
• Rest of the visualized abdomino-pelvic organs are within normal limits. • Retro peritoneum partially obscured by bowel gas; Suggested clinical correlation and Review with Clinical Findings for further Evaluation.

You have a large fibroid

Hello.
Thanks for writing to us.
As per your ultrasound scan you have a large fibroid in uterus but if you don't have any symptoms then there is no emergency to go for a surgery. These are benign masses that remain as it is for many years. But the fibroid can only be removed surgically. It will reduce the abdominal discomfort if any.
You can plan surgery as per your convenience.

Multiple thin walled loculated septate region in the right inguinal canal and extending into the right labia, shows slow flow within it ? Lvmphangioma . IT measures approximately 7.5 x 1.5 x 4 cms. Peritoneal collection is also seen in the right inguinal canal measuring approximately 3 x 0.5 x 1 cms.

What actually does it mean? And if will not do surgery is this will impact further..

Hello.
Thanks for writing again.
This finding is suggestive of a small lymphangioma. This is also a benign swelling but needs a color Doppler study and direct examination before any intervention. This is not likely to grow or impact fibroid. It is a type of vascular malformation. Not much treatment is needed in many cases.
