Good Day Doctor, Please I Have A Brother Who Has
Question: Good day Doctor, please i have a brother who has been diagnosed to right side hydro nephrosis pelvic ureteric junction obstruction and wanted know how to go about it treatment thanks
Brief Answer:
A stent is needed to correct the narrowing of the urether
Detailed Answer:
Hello and welcome at Ask a Doctor service,
Your brother has a dilated kidney due to a narrowing in the tube that comes out of it (urether).
It is a good news it is only on one side and the blood tests show normal levels of kidney function.
As for the treatment, it consists in treating the caude by dilating the urether. This can be achieved by a stent that is inserted there. This will release the pressure back in the kidney and help with hydronephrosis.
A good follow up with the doctor is essential for a good outcome.
I hope this answers your query. Let me know if I can assist you further.

Dr. Antoneta Zotaj, General and Family Physician

thanks for swift response. I will appreciate if you could suggest a doctor in Nigeria where we can get a good follow up please. What are they cost implication of treatment and his chances of surviving it . Thanks
Brief Answer:
Answer in detail below
Detailed Answer:
Hello and welcome back,
Thank you for your reply.
The cost of the surgery depends on the route that the doctor will do it: if he will do open surgery, through the skin, or through the urinary route.
Prices vary greatly from country to country. In the western world, this surgery would range from $15,000 to $20,000. I am sure in Nigeria the price for the surgery should be much less.
As for the best doctor to operate on you, this is a decision that is best taken with your doctor as they do know the system better. The local doctor can inform you how equipped certain hospitals are and which are the doctors with the best results.
To conclude:
- the cost of the surgery depends on the route that the doctor will choose
- prices will depend on the country, with much more higher expenses in western countries
- it is best for you to get the information on the best doctors and hospitals through your local doctor, as he should be quite familiar with the medical system locally.
I hope this answers your query. Let me know if I can assist you further.

Brief Answer:
No liver tests are uploaded, kidney function tests uploaded are normal
Detailed Answer:
Hello and welcome back,
I carefully checked the uploaded tests. The only blood tests done are kidney function tests and they are all within normal limits. So there is nothing to worry about the kidney function at the moment.
If you have the liver function tests, please feel free to upload them so I can review them.
Kind regards,
Dr. Antoneta Zotaj, General and Family Physician

Brief Answer:
Some tests and examinations are needed to know the cause of the increased liver test results
Detailed Answer:
Hello and welcome back,
I checked the liver blood tests. There is an increase in the bilirubin level and also the GOT and SGPT levels. Bilirubin seems the highest one.
There are several possible reasons for this increase ranging from a heart (congestive heart failure) problem to hemolytic anemia (break down of the blood cells), and liver problems itself.
Knowing more about the symptoms of the patient and having a few more examinations are needed.
- does he have jaundice?
- does he have shortness of breath, chest pain, and leg swelling?
- is his uring dark?
- has he had a CBC test and if so, can you attach the result?
- has he had a liver ultrasound, if so how was the result?
To conclude:
- there are several causes for the high liver tests
- CBC is needed to rule out hemolysis (red blood cell break down). Other blood tests might be needed as well, depending on the result of the CBC.
- liver ultrasound and a heart ultrasound might be needed as well
I hope this answers your query. Let me know if I can assist you further.
