Question: Hello! I suspected about a year ago I had a syphilis infection. I started feeling extremely anxious to the point where i felt as if i was going to pass out. I had lower back pain that seems to be around my lower spinal area. A red rash on my face. (Please note that all of these symptoms still occur). My doctor said that it’s just lower back back, seborric dermatitis and anxiety. I’m nervous these are all linked although she assured me they are not. I took it upon myself about a year ago to buy an at home std test. I pricked my finger and got as much blood as I could and sent it back. The test came back as reactive for syphilis (this was a rpr test) I immediately followed up with my doctor who gave me a Treponema Pallidum Particle Aggl Venipuncture. This test came back negative. I’m nervous that this has transpired into neurosyphilis and that is the reasoning for the lower back pain and “hazy anxiety” feeling. Between the time that I received that positive test and the negative test, I was treated with an awful lot of penicillin from a nurse with urgent care because I felt as if I needed treatment immediately. My question is, should I be concerned at all? Should I get another test from an outside source? And I overthinking this? Could all these things that are wrong with me linked? Is it all in my head? Thank you!

I have gone through your query and here is my advice.

As per your history and attached reports, RPR is positive and TPPA test is nonreactive.

Usually RPR test is a screening test and not a confirmatory test. It is also not specific test. There is chance of false positive test in other infection also.

So for further confirmation TPPA or FTA-ABS (fluorescent treponemal antibody absorbed test ) is advised which is a confirmatory test.

In your case, TPPA is non reactive, so I don't think it may be syphilis infection.

Still considering your typical symptoms and/or high risk/unprotected sexual exposure (if any), I would suggest you to repeat TPPA and FTA -ABS test after physician consultation to rule out STD.
