Question: I am a 36 year old overweight female. For the past few months I have been feeling extremely exhausted and my stomach constantly feels like it’s in knots. I have also managed to loose in excess of 50 pounds without even trying to do so. Within the past few weeks, I have started having lower abdominal pain. Sometimes it feels movement and other times like a persistent pinch on my lower left side. I am also experiencing what almost feels like a pins and needles sensation on both sides of my belly button. My biggest fear is that I could be pregnant and have not known it. I have read that obese women can loose weight during pregnancy if the fetus is not getting enough nutrients. If I would have conceived, it would have been at the end of October (7 months ago) I have had approx 15 negative home pregnancy tests, along with a negative blood test taken last week. The doctor did do a full panel of blood work to see if he could determine where my exhaustion is coming from, which appears to be anemia. I have continued to have what I thought was my period, but for the last 2 months it only lasted approx. 2 days and I have always been a 7 day person. Is it possible that I could be 7 months pregnant and still be testing negative?
Maybe it's hormonal or I am I just getting myself paranoid?
Could it just be my body adjusting to the weight loss?
I know that was sort of multiple questions, but any suggestions?

What is height and weight? - to know the BMI.
Any Ultrasonography, X-ray or CT scan of abdomen done?
Stool tests?
What is the hemoglobin level?
Any other symptoms related to Gastrointestinal system like nausea, vomiting, bloating, loose or hard stools?
Any urinary symptoms?
Have you been examined by any Specialist like General Surgeon/Gastroenterologist?

Anemia is certainly one of the most important reasons for exhaustion.
Since blood tests is also negative then there is no possibility of pregnancy; and X-ray/Ultrasonography will confirm the same and also may give idea about the cause of abdominal symptoms.
Stress and anxiety can instigate or enhance many of the abdominal symptoms if the above mentioned tests are also negative.

I am 5'5 and 228 pounds. Per my doctor, my hemoglobin was only down 2 points.

Within the last month, I was suffering from extreme constipation which in turn lead to bloating and gas. Also, leading to stomach pains. After 3 or 4 days I could finally pass a stool, but it was very small and loose. Things seemed to have improved where that was concerned.

I have also been under a lot of stress which could have caused the constipation.

I have not had any x-rays or ultrasounds. Again, my biggest concern was pregnancy and which it seems you are completely ruling that out. Can't blood and urine tests be wrong?

As per the information you have provided, your BMI (body mass index) is 37.9 that comes under the category of Obese. Hence this can be one of the reasons for your symptoms. You have to work to reduce your weight.

Stress as you have mentioned now can have direct effects on gastrointestinal system.

7 months of pregnancy suspicion, which you still have will be ruled out by Ultrasonography. It can also detect any other problem related to uterus, ovaries and many other things.

Correction of anemia will surely help you further.
Since one of the important symptoms are related to Gastrointestinal system an opinion of a General Surgeon or Gastroenterologist can really help to get an appropriate diagnosis thus getting proper management and possible cure.
