Brief Answer:
It can be due to several causes, needs further evaluation.

Detailed Answer:
Hello,
I have gone through your query and here is my advice.

As per your history there may be several causes for such symptoms, which needs further evaluation.

For that i would suggest you to consult a gastroentrologist, after complete medical examination he may procure you some investigations, complete blood count, stool analysis, colonoscopy, ultrasound abdomen, computed tomography abdomen.

Treatment depends on the results, till then you can take tab acetaminophen 5OO mg for back ache, can apply diclofenac gel locally.

Keep your self well hydrated, avoid stress, heavy and spicy food. Eat easily digestable light food.

Hope I have answered your query, let me know if I can assist you further.
Thank you.

Regards,
Dr Shyam Kale.
