Suggest Treatment For High SGPT And SGOT Levels
Question: Hi my wife is 28 years old.she is detected with high spgt and sogt level around 140/110 3 months back.we did ultrasound and found she have fatty liver.she started taking medicine.but the levels are always varying.some it will get reduced, then again it will increase.what should be the reason
On 20th of May the SPGT and SGOT were 32 and 65 respectively. But today it's 75 and 108 again. She was taking Ursodeoxycholic Acid tablet 150mg twice a day till last week. Last week doctor prescribed Ursodeoxycholic Acid 450 mg tablet . But the SPGT/SGOT levels are increasing.
Some doctors suspected it aa an aftereffect of pregnancy.our kid is now 8 months old. After de delivery,she don't have mensuration yet.

I read carefully all your wife concerns and I can say as follows:
1. You are reporting she has three months with high transaminase level and fatty liver.
Fatty liver can be associated with high transaminase level especially grade II-III.
2. I need to know more about her medical reports like:
- hepatitis tests like hepatitis A, B, C?
- did she use any medication before all this started?
- did she performed serological test for cytomegalovirus ,EBV, aoutoimune hepatitis?
If you have other exams of your wife please can you upload to me so I can check them by myself.

Tests for hepatitis B and C and Ceruloplasmin had taken .all the tests are negative. Before that no medicines had been taken. One pain killer tablet was taken for joint pain.but a single tablet only and avoided that. After delivery one aurvedic medicine was taken which contains fat.that was in last October .
test for cytomegalovirus ,EBV, aoutoimune hepatitis has not taken. Hematologist asked me to take liver function tests after two weeks for observing the trend.
I don't have soft copies of her reports. Below are the sgpt and SGOT levels of past few tests.

To my opinion she needs to do citomagalovirus, EBV and autoimune tests to exclude them.
Usually in cases like fatty liver, the transaminases level doesn't fluctuate as in her case.
You are reporting that she has had ultrasound on February, it is advised to repeat it again now. It is important to check about biliary sludge and fatty liver( if there is improvement).
I also advice to repeat the tests after two weeks and if it is possible to change the laboratory clinic.

Hello,
She has mega ureter condition and have two small kidney stones too.will it have any role in liver disease. Also if it is related with cytomagalovirus,will it affects baby as she is breastfeeding.

With regards to cytomegalovirus transmission with breast milk; yes it can be transmitted but, it is with no concupiscences on term babyis.
CMV infection with breastfeeding is problematic in premature babies.
