Brief Answer:
Could be due to infection of Testicle (Orchitis) or Varicocele .

Detailed Answer:
Hello
Thanks for your query.
based on the facts that you have posted I think your pain in testicle and lower back on left side could be either infection of the testis ( Orchitis) or Varicocele and bleeding while passing stool could be due to internal Hemorrhoids .

However this can be confirmed only on clinical and proctsopic examination by a qualified General Surgeon.

Please consult him and get following basic tests done to help to establish diagnosis.
1) Ultrasound Scanning of Scrotum.
2) Ultrasound Scanning of abdomen and pelvis .
3) Proctoscopic examination of rectum .
4) Colonoscopy (if required ).

Take antibiotics like Doxicycline with anti inflammatory medicine like Diclofenac and Metronidazole twice daily for five days .

Further treatment and any more investigations required will depend upon the results of these tests .

Hope I have answered your query .Please feel free to ask if you have more questions I shall be happy to help you .
Thanks and Regards.
