What Causes Frequent Urination And Difficulty In Emptying Bladder?
Frequent urination (usually at least once in the middle of the night)
Difficulty urinating (sometimes it is more difficult than others)
Difficulty completely emptying my bladder
Semen in urine sometimes
Ejaculation of semen after urination
Urinating more difficult when standing
Painful erections when sitting down
A bottleneck in the middle of my penis when cold (at times, this doesn’t always occur) This gives my penis an hourglass shape.
Weak erections (it’s like the tissue isn’t filling up with blood completely to make it rock hard. It is stiff but not as stiff as it can be.)
History
After puberty when I would masturbate I would often grab the halfway up my penis and squeeze very tightly as the organism came. I did this to prevent semen from coming out of my penis. I think this caused damage. So that would have been around 16-17 years ago. There was one time in particular when I did this that I felt something change and then I realized that what I had been doing was a terrible idea and I never did it again. It was after that incident however that the bottleneck appeared in various circumstances and especially when in cold water.
Question:
What likely happened? Is it possible to have something done that will correct this?

I read your question and i understand your concern. Holding semen episodes 17 years back is hardly and unlikely correlated with the recent onset of symptoms so dont worry although that habit is bad but its effects are not detrimental and everlasting and self resolves with avoiding repetitions.
As far as frequent urination and difficulty emptying bladder with dribbling of urine is concerned, these are common features of urinary tract infections , cystitis and urethritis. Its very easy to sort them out. A simple urine routine exam and a few days medication with good coital and toilet habits will fix things. Urinating while standing is difficult any how compared to sitting . If the urine routine exam comes clean i would suggest urodynamics studies just to be on the safer road.
Semen coming out after peeing is normal in sexually inactive people. As an ejaculate is ready on 4th day and with sexual inactivity , this oozing out is not worrisome and normal physiology.
However this painful erection should be evaluated for peyronie's disease a curvature problem in penis where too much curvedpenis leads to painfule rections, It happens in chronic masturbations and rough ones. Just let the doctor have a look, they will know whether its curvature problem or an association with urethritis.
