Question: Hi my 28 month old daugther test results shows calcium 8 vitamin d 4 and alkaline phosphatase 718. And her x ray report say rickets so doctor prescribe her ( injection ARACHITOL 6 LAKH UNITS for onces.and sup OSTOPOLYBION D 5ml morning and eveiningfor 45 .is this treatment is right .she has bowel legs .can i start sup OSTOPOLYBION D before injection or its compulsary to take injection first .because i am out of station .can i delay injection for 4 or 5 days . i am really worried about her low vitmin d and calcium level .i read that it lead to seizure .pls help

Hi....
1. You can start ostopolybion syrup before arachitol injection.
2. Treatment of arachitol is right... But instead oral 60000 IU XXXXXXX 1 per week for 4-8 weeks is also acceptable.
3. You can delay the treatment by 4-5 days if ionized calcium is normal. Get it checked. Only low ionized calcium will cause seizures.

2 arachitol injection and ostopolybion treatment is sufficient for all calcium and vitamin d deficency
3 if ionized calcium is low need different treatment.

Hi...
1. Yes ostopolybion can improve ionized calcium.
2. Arachitol and ostopolybion is sufficient for treatment. But there should be a follow up with your doctor and the vitamin D levels needs to be checked, if they have come to normal or not after 6 weeks.
3. Yes, if ionized calcium is very low, then I would prefer to keep the kid on activated form of vitamin called Rocaltrol orally and also give calcium not as syrup, but as intravenous calcium gluconate to bring up the ionized calcium levels to normalcy faster. Then I would proceed with the usual vitamin D deficiency treatment as recommended - like Arachitol injection and oral Calcium supplements.

Iodized calcium test facility is not available here so
Tomorrow I am going back to XXXXXXX
I will start her full treatment there

Her calcium level is 8.5
And vitamin d level is 4
By these reports Is her iodine calcium level can be low. she prone to seizures.

Hi...Please do not worry.
Her total calcium of 8.5 is low but not so low that she can develop seizures. It is highly unlikely that she will have a seizure because of low ionized calcium.

But there are instances in my experience that with this level of calcium too, very rarely there can be low ionized calcium and seizure. Please do not worry at all as this is very rare.

Moreover you have already started calcium supplements - so be rest assured.
