Your wife is suffering from pleural effusion left for which she has been put on tb treatment.
She seems to be responding well. In the x Ray report sent by you I really can't appreciate the swelling but there definitely appears to be some bunting of CP angle. It could be left over fluid or thickening of pleural membrane. You should have sent radiologists report on x Ray along.
The pain could be because of the pleural thickening as when the tissues stretch, there may be some pain. That's ok and she should continue deep breathing exercises and should even walk.
However if there is any infection in the left over fluid which has got infected can also cause pain. That then needs to be further investigated.
Why her tb treatment is being planned till May 17. Normally it's given for only 6 months if response is satisfactory. You may discuss these issues with her treating physician.
Do not worry about the ESR, it's ok as of now.

The treatment can be extended upto 9 months if the response to treatment is delayed. So why 1 year? There must be a reason. As i said earlier there is some blunting of left CP angle. On xray it`s not possible to say if it is fluid or it is pleural thickening. You have to get a ultrasound chest done to tell the difference.
