What Is The Ideal Dosage Of Cytomis Taken For Abortion?

Question: possible failed abortion. Used cytomis 200mg. 4 pills twice at 4 hour intervals inserted vaginally. Was +-6 weeks pregnant, bled a lot and had terrible pain and cramping. Passed large amounts of tissue averaging the size of a plum to the size of a lemon. About 12 large clots and varying smaller clots. Kept passing tissue and blood for a total of 12 hours then tissue, blood and pain ceased completely. 1 week later and nothing else has passed. Only a small structured piece of tissue passed with all the other tissue. It was quite round in shape and the size of a lentil, however it did not look anything like any sort of fetal tissue or sac. I can only see a doctor for a physical exam in about 10days or so. Could this very possibly be a failed abortion? what are the chances of infection if it is, since I cannot see a doctor in quite a while.

The dosing regimen you used to terminate your pregnancy at 6 weeks should have been effective, especially if you passed tissue. The fact that you stopped bleeding shows that your uterus is likely empty. A six week pregnancy is not very large and may not be easy to recognize of you were bleeding heavily.

The best way to determine if all the tissue is out from the pregnancy is to perform a urine pregnancy test. If it is still positive, you may still have pregnancy tissue in your uterus and may be at risk for infection. In that case, you really need to seek emergency care to get a prescription for antibiotics.

However if the test is negative, it means the medication worked and everything is fine and you can follow up with a physician in a few weeks as you planned.

Regardless of the result, you should refrain from putting anything inside the vagina for two weeks, so avoid tampons and sexual activity until your uterus has a chance to close fully.

Thank you so much for your response. I failed to mention that I did do a home pregnancy test today, 9 days after the abortion, and it was positive. However I have read that hcg hormones can be detected in a home pregnancy test up to 3 weeks after an abortion. Is it possible, in any way at all, that the pregnancy is still viable? That the fetus is still living? Or is this most likely, as you say, unpassed tissue?

There is a possibility that it could be too early to have a negative test... depending on how high your hormone levels were from the beginning. You could wait another week and repeat the pregnancy test and as long as you're not bleeding and feel fine, there is no danger in waiting until your doctor's appointment, when you should have an ultrasound to confirm that your uterus is empty. I think your chance of having a completely viable pregnancy at this point is very low and your diagnosis is most likely that the medication worked and your hormone levels just have not completely dropped yet. If you do have retained tissue you likely will start bleeding again, so be on the lookout for that.

I hope this additional information is reassuring and I think repeating the test in another week as long as you have no bleeding is the best way to proceed.

Thank you Dr. Brown. I have one last question. I am currently living in the Middle East where abortion is highly illegal and carries a huge penalty if caught. Doctors are required by law to report any person they suspect to have had an abortion. That given, is it in any way possible, via blood or urine tests or via an internal or external ultrasound, to tell the difference between an abortion and a miscarriage? As I would not be able to confide in a doctor here without extreme punitive measures.

I understand the gravity of your situation. Fortunately, unless you still have a live fetus, the doctor can treat you as if you had a miscarriage as there is no way to tell the difference. I think it is unlikely you still have a viable pregnancy. If you repeat your pregnancy test in a week and it is negative, and you feel fine, you really don't need to see the doctor at all as it means the medication worked completely.

I hope that everything works out. Please be in the lookout for increasing abdominal pain, fever and foul-smelling discharge as these can be signs of infection.
