Is Surgery Needed For Clot In Brain And Ears?

Hello dr.
My brother met with an accident on 17/04/2010 resulted in head injury.He was unconcious for 3 days n after MRI internal bleeding n fracture. His right ear was bleeding continously for abt 7 to 8 days. After 4 days again MRI was done which showed that his fractures r filled. After abt 15 days he was discharged. Then he continued his treatment from neurophysician who prescribed him nootropil 400 mg twice a day for a month n that time he had 6.2 mm blood clot in his brain. Now after 15 days he had giddiness, head pain n vomitings. Then after that he was admitted to hospital n after MRI again his blood clot has increased from 6.2 mm to 7.4mm. My brother 17 year old said that it's due to that he even have blood clot in his right ear which continously moves when he turns his head n leads to little bit giddiness.
Now he has referred to a neurosurgeon n ENT.
Will he has to get surgeory or medicines?Is it fine if he's treated by medicines then v shud get him discharged
