Hi,
Going through your query, I have one big doubt.
ELISA is done for which infection? AIDS?

Night sweats can be seen in tuberculosis but also can be seen in other inflammatory conditions and cancerous conditions.
Loss of appetite can be seen in chronic conditions.
Patient with diabetes could have other chronic infections.

Specific diagnosis is necessary before ATT is started.
A normal ESR and non diagnostic x-ray is against tuberculosis.
My advise is not to start ATT.
If it is AIDS, culture and sensitivity is necessary to look for resistance to antibiotics.
Any further queries, happy to help again.
