I am married for three years. On the first year of my marriage, my wife conceived and a baby boy was born on 26/10/2007. At birth, he was absolutely normal. Nevertheless, 21 days after he developed a chronic pneumonia, which local doctors failed to diagnose, and finally he had to be admitted to a reputed nursing home in South Kolkata. Though he was critical at the time of admission according, he recovered gradually and when he was declared out of danger he was brought home. After ten days he conditions suddenly deteriorated and had to be admitted again to same nursing home. After some days it was found that the baby was oxygen dependant. As time went by his oxygen dependency did not decrease. Bronchoscopy was performed on the baby and no deformity or any abnormality was found. Then consulting with other doctors my son was administered Penegra. After a few days the doctor told us that since the baby’s oxygen dependency did not show any improvement it was a clear case of ILD from which there was no recovery. The baby often reached a stage of cyanosis and he died on 24/12/2007. After this incident we decided to have a thorough check up of my wife under supervision of a reputed gynaecologist. The following tests were performed and the results are shown below: APTT Patient plasma 32 sec. Normal values: 22 – 34 sec. Control plasma 24 sec. HORMONE ASSAY T 3 1.75 ng/mL 0.8 – 2.0 T4 10.58 µg/dl 5.1 – 14.1 TSH 23.32 µIU/ml 0.27 – 5.0 TORCH Toxoplasma(Gondii) IgG 5.7 IU/ml 3.0 = Positive Toxoplasma(Gondii) IgM 0.128 index value 1.2 = Positive Rubella IgG 1.0 IU/ml 5.0 – 9.9 = Equivocal 10 = Positive Rubella IgM 0.141 index value 0.6 – 0.7 = Equivocal 0.8 = Positive CMV IgG 250.0 AU/mL 15 = Positive CMV IgM 0.262 index value 0.400–0.499 = Equivocal 0.500 = Positive HERPES IgG 12.5 N.T.U 11 = Positive HERPES IgM 4.40 N.T.U 11 = Positive ENDOCRINOLOGY Prolactin 8.11 ng/ml Not pregnant: 5 – 25 After examining the reports our doctors gave us nod to go ahead in case we wanted a child. Since after caesarean section delivery and after a previous shock she was quite weak we decided to wait for a few months. Our doctor gave my wife a contraceptive pill (Novellon). After a few months my wife again conceived. During pregnancy my wife took Folvite, Gestone 200, Shelcal 500, Doxinate and Globac Z. The baby girl was born on 29/03/2009. However, four hrs after the delivery she had to be admitted to a nursing home after reported difficulty in breathing. To our utter dismay it was found that the baby has developed a chronic oxygen dependency. After this the baby was shifted to Institute of Child Health. Here her condition improved gradually and after a few days she remained stable though she required a minute amount of oxygen. From now on the baby could not live without oxygen and her condition fluctuated all the while. From then on many tests were carried out. One of the was the echo cardiogram test but that showed no such abnormality. USG of the brain showed no abnormality. Though initially the X – ray reports of the lung showed opacity but later reports suggested “Hyperinflated but clear lungs, well defined diaphragm and costophrenic angles, normal hila and heart size shown. Bony and soft tissue shadows are normal. Clinical correlation suggested”. Despite sincere efforts by the doctors of ICH no diagnosis could be made about the cause and doctors again suspected this to be a case of ILD. On 17/06/2009 the baby died. Needless to mention that such a situation where no diagnosis is possible it is quite frightening to plan for an issue in future. Hence I would like to know whether is possible to ascertain the true cause of this incident so that such a situation does not arise in future. Thanking you, Yours faithfully Aniruddha Bhattacharjee
