What Causes Blood On Tissue While Wiping After Urination?

I am 28 years old and I had a total hysterectomy 3 years ago. Yesterday, after I peed, when I wiped the second time, I started to bleed bright red blood. I wiped again, and then once after that, and the blood was still there. I have not noticed any more blood since then. What could this be? Could it be because I wiped too hard?
