What Causes Frequent Headaches?

I have had severe headaches for over a year. My family doctor has tried several medications such as imitrex, naproxen, ibuprofen 800, ultram, fioricet, maxalt, midrin...none of these medications help. I have already had a CT scan which was normal...why do I keep getting headaches like this and how can I make the pain go away?
