What Causes Miscarriages Due To Amniotic Fluid Leakage?

I have had two miscarriages at 19th week, both normal delivery. My amniotic fluid started leaking in both the cases. 1st time, I lost the baby the day I started leaking fluid, and 2nd time, I lost my baby after being on medication and bed rest for 17 days. I have done couple of tests which have been negative so far. Only my Insulin level is too high.
