What Causes Severe Knee Pain?

weight: 68 Kgs
height: 162 cms
masturbating from last 10 years with frequency roughly thrice a week presently.
smoking habits with 5 ciggrates a day
knee pain.. sometimes in right and sometimes in left... feel like a sweet pain continuously in both the knees. Had both x-rays and MRI's but nothing detected...apart from this , whenever I involve myself in a bit of physical activity, I start to pant.
