Hi, I Am 33years Old Sainy. From The Last 12

Hi, I am 33years old sainy. From the last 12 years I am facing a problem. Whenever I become pressurised my hands start shivering, my voice become stuck in my throat, my blood pressure become raised, I feel panic, palpitation raises. I feel a mountain on my head. I feel that I am going to die. Despite of much courage I even can not move from my place. In crowd also same condition. Please help me
