Hi, I'm Just Wondering About Something. My Knee Cap Is

Hi, i'm just wondering about something. my knee cap is out of where it's supposed to be (permanently), and whenever I take a hot shower or bath it makes my knee but a lot worse, to the point where I can barely. end it. Is this due to the kneecap problem or us there something else as well? also, I don't have a doctor since I moved and I don't really go to one anyways, so I hope you have an idea other than "go see your doctor". Thanks!
