I Believe That I Have A Rotate Cuff Tear And

I believe that I have a rotate cuff tear and a tear in the tissue or muscle below the shoulder about 8 inches but what I am concerned about is the possibility that I have also torn some tissue straight across my chest is that a possibility....I have a very physical job pushing 150 pound metal boxes on wheels and lifting 50 pound trays over my head......I have some pretty intense moments with pain after a long day's work but when I have some time away from that work the pain decreases with Ibuprofen/ice/heat...would this heal on its own? I can't afford a doctor as my deductible is 2k,,,,,
