I Feel Dizzy, I Have Insomnia, I'm Feeling Weak, Tired,

I feel dizzy, I have insomnia, I'm feeling weak, tired, confused. I'm not depressed, I do heavy work with help, I'm 50 years old and live on a farm and take care of animals, but I can't seem to do normal things like I used to. I get headaches and ringing in my ears here and there. I fell off my horse and had a concussion and went to the emergency room a year ago, and a couple weeks ago I fell of my horse once more and my neck usually hurts.
Please help, I miss spending time with my children and going riding with them.
