I Have A Suspicious Birth Mark On The Right Side

I have a suspicious birth mark on the right side of my back between my shoulder blade and spine. The skin has no pigment and is a little bit smaller than a quarter. It has always gotten burnt in the sun for 25 years. Recently a year ago, I noticed it would get red, puffy and flakey. Heal and repeat, then dark spots showed up one at a time, kind of like moles but more patchy looking. The area hurts, about 4 inches in diameter of the birth mark. It's very tender and now my back feels a severe stabbing pain frequently that nearly knocks me down from sudden pain. It tends to linger for hours. I saw a dr and she simply said it seemed very odd and recommended derm. But she didn't say whether I should see them immediately or if it can wait till an available appt which isn't for 6 weeks. Would you suggest a more urgent appt for my case?
