I Think I Have POTS - My Doctor Said "blood

I think I have POTS - my doctor said "blood pressure tendency to drop with positional changes, stress response" in the last visit summary, but I don't know if that actually counts as a diagnosis. I have been randomly going limp, hyperventilating, and occasionally fainting, since about the beginning of puberty. I never managed to figure out what it was from a multitude of doctors and emergency visits. I could collapse in public, but by the time the ambulance arrived I was coming out of it, so my readings would be relatively normal by the time I got checked. I know that POTS is something that qualifies for a service dog. Do you know what I can do to qualify for a service dog? I have a dog and would like to train him myself, but I'm not sure of the exact process and documentation needed.
