Should Fentanyl Patch Be Increased For Severe Pain In Back And Shoulders?

hi, Ive been on the fentanyl patch for 5 years, for severe back pain (bulging disc, herniated, etc.) and a shattered r knee (didn t want any more surgeries, had 4, so no knee replacement). I am now taking 1 100 mcg plus one 25 mcg patch every 48 hrs, with percocet for break thru. It sounds like alot, but Im a large gal with osteroporosis as well, and recently had an MRI for shoulder pain, so now I have a torn tendon with other tears, etc. there. Im 58 and just recently approved for SSD. This dosn t seem to be enough, because I ache all over(OH fibromialgia as well, take lyrica), and can hardly walk (hang on to furniture), so Im really in pain all the time. My only relief is to lay in a fetal type position and sleep, which I do too much. what should I do? Up the patch? Take a stronger pill for break thru, or roll over......... and die of pain.
