What Causes Chills And Rapid Heart Rate Along With Epigastric Pain?

My wife is normal height/weight 5. 3 , 155, good health, exercises etc. No meds, no allergies to meds, hx of GERD. About the 28th of Jan she had cold like symptoms and started feeling better on the 5th. She has continued to have a slight cough and sinus drainage. However, now it feels consolidated in her throat or upper epigastric pain that is different than her GERD pain. Her heart will race 104 now, high for her. She has SOB climbing stairs and occacionally sitting and after she coughs the pain is better for a bit and heart slows temporarily but she always feels pressure there. I listened to her lungs and her right middle lobe is decreaased but clear everywhere else. No fever. No Productive cough. Not sure what next? She doesn t run to the doctor or like to take meds. She had some chills today sitting on the sofa with a blanket. That s a lot. Thanks!
