What Causes Constipation Despite Being On Laxatives?

Hello, my wife is unable to poo, it could go on for 3-4 days before she is finally able to pass anything, and even then it is only a small amount and takes a lot of effort.She has tried laxatives, not straining on the toilet, staying on the toilet for a while, nothing seems to work... what can be done?
