guidance for my father aged 64 weight 55 is suffering from diabetes type 2 he is talking combination of metformin500
Glimipride2mg
Piaglitszone 7mg
And vigaliptin 50 mg and metformin500
And voglibose 0.3mg
In morning and evening before meals
Still sugar is 230 in morning after meals is its 250 no problem of bp hba1ac8 %when total intake of metformin was 1500mg in a day it was in control please guide also we are not able to get combination with Piaglitaszone

I have gone through your query and attached image. Here is my advice.
As per history and attached image, it seems that his sugar is not under control.
As you said when total dose of metfomin is 1500 mg day than sugar is under control phase. So I would suggest to take that tablet metformin dosage after diabetologist consultation.
Tablet metformin 1500 mg per day is a safe dose. Along with that considering his age 64 yrs, tablet metformin is safe antidiabetic medication as it rarely causes hypoglycemia/low blood sugar level.
If still sugar is not under control, again considering his age, it would be better to avoid tablet pioglitazone and start injection lantus insulin (long acting) at bedtime after diabetologist consultation. You may require daily blood sugar level check up at least 5 times a day.
Watch for hypoglycemia/low blood sugar level symptoms sweating, giddiness, fast heatbeats, shakiness, hunger.
Along with that kindly follow diabetic diet. Do regular exercise for at least 20 to 30 mins.

Thanks sir,earlier medicine dosage was
combination of metformin1000 mg ,pia7.5,Glimipride2mg ,tablet acarbose,vigaliptin50 with metformin500 twice a day .when we check sugar at 7 pm it is upto 150 to 180 ,sugar hikes up in the morning only,earlier when doctor prescribed insulin,his sugar gets low,please guide at what intervals should the sugar test be done,also if any medicine is to be added or we should continue the same dosage as earlier written above,when he was taking triazulix forte tablet with vildagliptin 50 metformin500 sugar was in control,now we are not to get such combination in market.please guide us for the medicine ,so that fasting sugar gets in control, regular exercise is being done, also he complains that he has lost the taste of mouth doesn't feel hungry and gets bad dreams, please guide for medicine

Considering current covid pandemic situation, covid 19, candidiasis, multivitamin deficiency, oral/dental infection may cause loss of taste. So for detailed clinical examination, I would suggest you to consult with physician.
As you said, he doesn't feel hungry and if appetite is decreased than anti diabetic medications dosage should be adjusted according to blood sugar levels.
So kindly do blood sugar testing at least 5 times a day i e before breakfast/lunch/dinner and 2 hrs after breakfast/dinner.
After complete recovery, If his blood sugar levels are under control with daily 1500 mg metformin than continue the same.

Thanks for your guidance, is their any medicine better than vigadliptin, or any other class of gliptin or which is long lasting because one of the doctor prescribed tenegliptin,while other prescribed vigadliptin.which gliptin should be taken for better results.
