Hello there,
Welcome to 'Ask A Doctor' service.
I've gone through your query in details.
I can understand your concerns.
As per the picture attached by you chances of it being hernia are quiet high.A hernia occurs when a small part of abdomen protrudes out of abdominal cavity.
Belly button going a bit out is again suggestive of umbilical hernia..Proper physical examination should be done by a MD.
Test suggested
Usg abdomen
CT scan of abdomen

Possibility 2-Formation of large amount of gas causes inflammation in intestine. This gas compresses the nerve passing through intestine hence causing pain radiating to back.

Go for the tests suggested along with proper physical examination.
Follow up with reports.
Hope I was helpful,
Thanks!

Hello,

Thank you for your answer.
I wanted to share with you also the blood test (test.png) and check if this would suggest other possibility.
Also, do attached results say anything about the state of pancreas or colon or different blood test would have to be made?

Thank you.

Detailed Answer:
Hello again and thanks for follow up my dear patient,
Have gone through your reports(though a bit difficult as some terminology used are not in english yet i can under stand almost every test result).

1)You are dyslipidemic your cholesterol levels are high and bad cholesterol called LDL levels too are high.Your cardiologist would put you on medicines called statins like Rosuvastatin 5 to 10 mg per day for minimum of three months.
Along with it he/she would recommend you to do exercise moderate in intensity at least 5 days a week.
Your pancreatic enzymes the last three in reports are normal.

Your liver function tests(some enzymes secreted by liver are mildly elevated may be due to fat deposition in liver called fatty liver).Get an ultra sound of abdomen test done to rule out fatty liver.
Take a liver rejenuvating supplement called milk of thistle or sylimarin once daily for 3 months. This will help to bring back the level of enzymes to normal.

Apart from that there are crystals in your urine. So again an ultrasound would help to rule out kidney stones.

Rest all other values are normal or have no significance.
Suggestion
Get an ultrasound abdomen done.

Hope i was helpful,
Follow ups are welcome,
Thank you!

Brief Answer:
Follow up.

Detailed Answer:
Hello again,
No I don't think that keto diet is medically a good option.
As a doctor my patients diet should be 50% complex carbohydrate 30%proteins and 15-20%Fat.
I am not taking about any sport or body building but as per any standard medical book which we study in our medical school the diet given by me is prescribed.Fats are much more difficult for body to digest as compared to carbohydrate therefore in any liver disease patient is given high carbs diet.
From my side don't play with body.No keto diet eat a well balanced food.
Hope I was helpful,
