Question: 2cm complex cyst found with thick walls and increased vascularity. What are the chances of this being malignant? I am having lower abdominal pain, frequent urination, pain during sex, back pain

I understand your concern.
Mostly small sized cysts (less than 5cm)with clear borders and in menstruating age are more likely not to be malignant & likely to regress spontaneously.... Moreover complex cysts are more likely to be benign.
possibility of ovarian cyst being malignant increses with ,age (around menopause) / history or family history of genital or breast cancer/increased vascularity and fast growth of the cyst along with less appetite &loss of weight.
Pains you have are mostly pressure symptoms.
In your case size of the cyst is small but increased vascularity & having pain (pressure symptoms) demand scrutinising possibility of malignancy by -
-Tumor marker test
- Repeated follow up every 15 days for increase in size of cyst , by ultrasonography scanning.
