Question: 31 year old male, 6ft 2 239lb. basometabolic labs all normal, echocardiogram normal, no allergies to food or medication. Stage 1 hypertension mitigated with Losartan potassium 50mg. Normal diet, no caffeine or sugar supplimentation. a1c's all normal and non-prediabetic, 80-106 blood glucose level average. Electrolytic panel normal. Operation history: gallbladder removal, tonsilectomy, hernia repair, upper endoscopy and colonoscopy. Symptoms onset within the last year only. symptoms include: fatigue, trouble breathing, dizziness, irritability, faint feeling, nausea, symptom-induced panic. Heart does have a minor premature ventricular contraction history. minor pain in upper left shoulder. Sometimes a tightness on the left side of the throat, a feeling of flushing or draining, in the head and what feels like a minor tension band headache. patient is hydrated and well rested. Symptoms all have a sudden onset and peak within an hour, however all lab results show normal. Every doctor I've spoken to passes it off as anxiety, when I have no medical history or family history of anxiety. Looking for potential further answers.

In my view the cause for your present symptoms can be-
- Impaired blood supply to heart muscles due to blood pressure & changes in heart blood vessels (Which are easily manageable with timely diagnosis,needful medical help &self care.
-The Anxiety could be causing these symptoms or it can cause the symptoms too.
*In this scenario I would suggest few cardiac investigations like stress test & angiography if needful along with complete physical check up & investigations for your general health.
Stress management is another essential thing you have to care for.This can be by your own efforts for positivity -stable psychology by mind concentration exercises OR with expert guidance & help by a psychologist /psychotherapist.
Management of physical -psychological aspects would help you to be out of your problem.Please don't worry.
