36 Year Old Female - Ultrasound Found Right Ovary Normal
Question: 36 year old female - Ultrasound found right ovary normal in size & appearance measuring at 3.2x1.5x3.3 cm.
Left ovary measures 3.2x2.5x2.6 cm. Mildly complex cystic lesion is present within internal septation measuring 2.6x2.1x2.2 cm.
No free fluid.
Family history of Ovarian cancer is a major concern (Mother died from Ovarian cancer). Cancer in general is found on both sides of family.
Can someone help me understand the ultrasound findings of the left ovary?
Should I be concerned about the “mildly complex cystic lesion” that was found?
The lesion found measures almost as big as left ovary. Is that normal?

To clarify my question: I’ve experienced severe lower abdominal pain that’s focused on left side. Pain from lower left abdomen will shoot down my leg towards my knee, that has been off & on for several months. Pain level is intense everyday, some more tolerable than others.

Brief Answer:
Doesn't appear to be due to cancer
Detailed Answer:
Hi
Thanks for your query.
Such types of small mildly complex cysts are usually benign in premenopausal women. They may be physiological or sometimes due to endometriosis. Endometriosis can explain your pain too. Ovarian cancer is usually painless when there's only an ovarian cyst and nothing else.
So at this point I don't recommend any further evaluation for it. However, given the family history, you could go for cancer genetic counseling.
Hope this helps.
Regards

I had the IUD removed today, which was not easily done. They had to use use a hook like device to pull it out. I’ve had severe abdominal pain everyday since November 2020. Pain so bad I’m in the fetal position praying for it to go away. Pain will subside usually within a couple hours & I then I can function normally. I contributed the pain to the IUD, but I’m not so sure anymore. What tests should I have done to confirm why I’m having so much lower abdominal pain? Since 2020, I’ve had several UTI’s & bladder/kidney infection. I haven’t had any issues like that for a decade. Something is going on & I’m growing tired of being miserable more often than not. I don’t want to go to the hospital unless absolutely necessary.

Brief Answer:
Yes it may be pain due to IUD
Detailed Answer:
I agree that something is going on. But it's unlikely cancer. Let's see what happens to the pain after IUD removal.
If pain persists, a CT scan of pelvis may be done.

I agree cancer is not a main concern here. Thank you very much for your informative words.
I start taking Macrobid for UTI today (burning sensation when urinating). Frequent UTI’s are out of the blue as well....
What is a decent time frame after removal, for the pain to decrease?
I’m in the same amount of pain today, as I was when I had the iud (has only been 1 day since removal).
I can feel my left ovary when I’m sitting or standing....it’s almost as if I feel it moving & that’s what hurts. Bloating seems to be an issue today.
I’ll wait however long I need to wait for things to heal and for pain to subside, but I would like to know how long that process typically can take. Thanks so much for your time....I desperately want to feel normal again.
