40 Years Old Male Smoker 25 Years Epigastric Pain Point

Question: 40 years old Male Smoker 25 years

Epigastric Pain Point Tenderness Pain right side pocket (space between rib cage)of sternum for 3 Months.

I can tell after drinking alcohol it feels inflamed or just way more sensitive to touch.

Water Bowels on and off depends if I drink alcohol. After a couple days without alcohol soft stoll but yellow.

Weight loss- 7 pounds 2 months

Feeling fatigued

Back Pain -vague on and off back right side rib cage.

Loss of appetite just not hungry depression

Blood tests a month ago clear.
Abdominal ultrasound clear
Lipase test clear
No Amalyze Test
No tumor marker test

Three weeks ago GI Doctor said I have Xiphoid Process. My symptoms do not sound like Xiphoid process.

I believe I have pancreitic cancer!

Hi,
Have gone through your query,
Going through your history the symptoms are because of Gatro esophageal reflux disease (GERD) with gastritis,The first possibility is of Peptic ulcer disease or Hiatal hernia. Diagnosis can be confirmed by upper GI endoscopy.It is a chronic digestive disease. GERD occurs when stomach acid or, occasionally, stomach content, flows back into your food pipe . The backwash irritates the lining of your esophagus and causes, heart burn,chest pain and intense burning sensation in throat.Both acid reflux and heart burn are a part of GERD.The presence of gastritis has proved additive to your symptoms.
Advice
Ask your gastroenterologist to increase the dose of pantroprazole or esomeprazole,Pantoprazole can be used in dosage as high as 40 mg twice daily ,Ash him to add Domepridome 30mg slow release once daily,This will slow down the reflux of acid back and would provide symptomatic relief.An antacid containing local anaesthetic could be added to drug regimen.Acotiamide is a new drug and is very effective in controlling symptoms of GERD and esophagitis.In trials- Acotiamide,a gastrointestinal motility modulator,at a standard dose of 100mg thrice daily has significantly affected esophageal motor functions or gastroesophageal reflux in healthy adults.
All these drugs can be considered by your gastroentrologist to provide you with symptomatic relief.

Apart from medicines few other life style changes are adviced-
- Increase warm water intake
- Do not drink water during and after meals
- Eat small meals (rich in fiber),Avoid spicy,oily and heavy meals.
- Eat plentiful "fresh oranges" and "pear "
- Drink COLD milk and and AVOID junk food.
- avoid beverages / alcohol/ soft drinks
- do some exercise early in the morning
- avoid carbonated drinks

I suggest you to consult a physician and get an ultrasound upper left abdomen, pancreatic enzymes and CT scan of abdomen done to rule out panvreatic cancer. upper GI endoscopy should be done to rule out any peptic ulcers

I understand what you’re saying. I guess with the omperazole and Naproxen that has been given to me has not done much. I don’t really have any burning. I just have point tenderness pain in the epigastric area for the first couple months and know when I move around I can feel the tightness right side of sternum.

The last 7 days all water diarrhea and more weight lose. I’m feeling fatigued and when I drink water I’ve to belch.

It blows my mind how this Xiphoid Process was even a factor in my current GI’s diagnosis.

I took a deep dive in artilcles on Internet and patient feedback’s stories and they all speak about Pancreatic Cancer.

I did have an ultrasound theee weeks ago and came back clear minimal view of Pancreas.

They lipase Test was normal but all the info from Doctors on Internet say that has nothing to do with Pancreatic Cancer markets.

I will have to wait for insurance company to approve CT, most likely a couple weeks.

Where I’m at mentally is my epigastric Pain today is worse than three months ago and everything I eat as of late goes right through me.

Have you tried probiotics ever?
Going through your history chances of it being pancreatic pathology is very rare rather dyspepsia can describe the symptoms very well.Dyspepsia is an umbrella term used to cover patients with symptoms of indigestion overlapping with that of gastritis.
I would suggest you to take tab imodium 4mg for two days twice daily.Along with it start taking a good probiotic like Econorm thrice daily.It would improve the overall digestion.
Get yourself physically examined by a sports medicine doctor to check if soreness at xyphoid process is somewhat related to musculo skeletal system.

Hi again,
Try the following things,
Less fiber in diet(floating stool is suggestive of very high fiber intake.
Gastritis usually cause epigastric pain radiating below rib cage.
Belching and bloating is again suggestive of incomplete digestion of food.Incomplete digestion cause liberation of large amount of gases.

You would feel much better in 3 days.

OK.

I have not been eating much as of late not much fiber at all.

I just went to the washroom again and checked body temp.

97 degrees feels like I have a fever and very fatigued
Just went to the washroom my urine was bright orange and smelled awful
My stool again was yellow floating with red in it
I have no appetite
In the last week I went from 186 to 178
Yes, the epigastric pain is still where it is at.

I'm not questioning you, I'm just digging for answers. I was able to get a CT Scan
next week but I'm in such panic right now this is all I have to talk about.

I need to fly out for work but I'm half anxiety stricken.

I know Pancreatic Cancer is rare but my symptoms are real, right?
