I keep waking with shortness of breath and a feeling of tightness around my chest. What could this be?

I torn my left shoulder blade about 2 years ago and it never healed properly.

Actually one of the crucial symptoms of anxiety (panic attack) is that patients get short of breath with chest feeling tight and they feel like they are suffocating. Sometimes this causes the patient to even faint if it lasts for a while.
Other symptoms that may happen with panic attack is palpitations, shakiness, numbness, etc, and different patients may show all or a few of these symptoms. Most of the time it is worse at night and rest.
They generally last few minutes to half an hour, rarely may go on for longer.
To conclude:
- the shoulder pain may contribute indirectly by favoring anxiety for to chronic pain
- because of the chest tightness at rest, is the very reason I am considering panic attack especially if it not affected by laying flat
- if you have worse chest tightness when you are laying flat and better with pillows then a heart and lung issue should be ruled out, chest x-ray is needed in this case. This would cause tightness every time you lay down, even during the day and you would also have swollen legs
- so if no swollen legs, no chest tightness every time when you lay down and if it not affected by the number of pillows, this is very likely a panic attack.

Just to let you know that I did originally wrote that I had a chest xray, blood work and an ECG a day ago.
All came back with normal results.
Would this indicate that this is not a heart problem?
Do you feel I would need a stress test? Only asking because the doctor mentioned it.

Especially if you have still your menstruation, you do not have diabetes, hypertension, high lipids, or you are not obese (BMI over 30) the chances are very little the stress test will be positive.
By what I understand even the ER doctor was not sure you really needed the stress test. Still because it does not hurt (it does not cause radiation or other dangerous effects) you may have it done and make sure.
I have explained some of the characteristics of the panic attack? Do your symptoms seem similar to that? Do they fade within half an hour?
Another rare condition that might be related to the heart in younger women is prinzmental angina, which is spasm of the blood vessels of the heart which happens mostly at night. In this case diagnosis is difficult because the blood vessels are normal and will show normal in the ECG and stress test but these women respond well to calcium channel blockers. Many think this is related to stress as well.

You may choose to have the stress test, just to have peace of mind, very likely it will come out normal

I need to have a tooth extracted on Saturday. Due to heart worries, would you think that it is okay to get the tooth pulled and no worries of HA?
I only ask because of possibilities such as Prinzemetal angina.
Also I do have a tooth absess, would this cause any of the above?

A tooth abscess would cause jaw pain, swelling locally and maybe fever, rarely the infection may go in the blood and complicate with endocarditis (infection of the inner layer of the heart) but if it causes chest pain it is continuous chest pain worse with breathing, fever, joint pains, and get progressively worse. It is important that you do the right treatment of the tooth abscess though, maybe your dentist will give you some antibiotics before working on your tooth.
So as for the heart the dental procedures in your case are safe.

I got a letter today from an internist. Apparently the ER doc I seen a few days previous set an appointment up for me, although it won't be for another month.
I recall him saying I might consider getting a stress test, so wouldn't he have wanted me to see a cardiologist? Why am internist? I don't understand.
I was also wondering, once again going back to my dentist appointment. Do you feel that it is safe for me more on the grounds of me not having any heart issues more due to being over stressed?
I do suffer from Fibromyalgia and I have fatigue quite severe most days. I am out of shape and although I am 5'4 and 120 lbs, I am a healthy weight, I do feel that my body is quite weak most days.
I try to move around during the day but it's mostly around my apartment and I tire out very easily.
I am now obviously concerned about this possibly heart issue.
What worries me is if I go to this appointment, use energy I don't have and cause myself heart issues due to that.
Or do you think that being as I had tests recently and all were fine that I am worrying for nothing?
I do get pains in my chest and back but I am quite certain those are costochondritis as I do suffer from that as well.
