Question: 54 yo female. 35 years working as a dental hygienist - very difficult to keep proper posture. Pain started in 90's and been managed with chiro, physio, massage etc. I get heavy arms, right arm can get cold (occasionally) lots of back pain, sometimes muscular, sometimes bones. Pain comes thru chest at times. Wakes me up at night, not good staying on back. MRI: mild multilevel degenerative disc disease. No compression fracture. Incidental not of several vertebral body hemangioma No significant spinal canal stenosis or neural formaminal narrowing. Mild posterior disc osteophyte complex at T5/T6 causes minor impression on the spinal cord anteriorly.
Does this mean osteoarthritis? how is arthritis in back or neck diagnosed?

Brief Answer:
Looks like spondylosis

Detailed Answer:
Hello and welcome to Ask A Doctor service.

I have reviewed your query and here is my advice.

Back pain, thoracic pain , or joint pains are classified as inflammatory/ non inflammatory.
in case the pain are worse in the morning after rest, or in late night, and the pains wake you up from sleep typically at 3-4 am, are associated with stiffness which is prolonged lasting more than an hour after you wake up, the disease is classified as inflammatory, diseases like spondyloarthritis, which require specific treatment.,

If these are non inflammatory or degenerative typically pain is worse after a day of work, they are better after rest and there is minimal stiffness. the treatment remains rest, strengthening exercises for the neck and spine, supplementation with calcium, vitamin D in case deficient, physiotherapy and sometimes medication to take care of the nerve pain and heaviness like pregabalin.

So osteoarthritis is a kind of degenerative joint disease, clinically I told you how it presents, X-rays and MRI confirm discs, compression, impression, etc implying degeneration.

is degenerative disc disease the same as osteoarthritis? Are the names interchangeable?
what is the difference between impression on spinal cord from compression?
The MRI report doesn't speak of disc compression but a broad "multilevel degenerative disc disease" term.
How is the distinction between normal aging signs made from actual damage?
What causes it to be inflammatory?

Brief Answer:
Seems degenerative

Detailed Answer:
hi degenerative disease is similar to osteoarthritis.
compression generally is more significant than impression.
it means the cord or nerves are being damaged.
there is no such thing as normal aging, some people may have more degeneration than others.
inflammatory arthritis is different, usually autoimmune.
