Question: A 72 yr old was incoherent with slurry speech while having evening tea. There was slight delay reached the hospital. The current CT scan shows the image attached. The BP recorded after the event was 220/??. She is put on ventilator and in coma. She had history of high BP. A past history of MI with two angioplasty with stent in 6 years back. What is the prognosis?

I have noted the clinical details and seen the attached CT image.

There is a large brain hemorrhage (bleeding) on the left side of brain, involving thalamus and internal capsule with extension into lateral ventricles. The bleeding has occurred due to high BP.

Surgery is needed as a life-saving procedure. Mortality and morbidity remains high, especially if surgery is not possible due to poor general condition of the patient.

BP should be rapidly controlled with Labetalol injections.
