Question: AST and ALT enzymes are elevated. AST is 179 and ALT is 300. BMI is 37.5. Does this sound like fatty liver?
Brief Answer:
Enzymes are only borderline elevated.
Detailed Answer:
Hello,
Thanks for the query.
The enzymes are only borderline line elevated. These are not significant and does not indicates any active diseases or fatty liver.
You can go for an ultrasound abdomen to look for features suggestive of fatty liver(in fact fatty liver is an ultrasound diagnosis and not a liver function based diagnosis).
Restrict the use of alcohol as it will worsen the general health of liver.
Your BMI is very high and thus you are prone to develop liver dysfunction.
Hope I have answered your query.
Thanks

Are you sure they are borderline elevated? UNL for AST and ALT were 40 and 56 respectively so they seem very elevated. My doctor said he is suspecting fatty liver but those numbers are high for that.

Detailed Answer:
Hello,
If there is major liver diseases the enzyme values will be in thousands.
Fatty liver is suspected, when there is progressive increase in liver enzymes or the high levels persist for a period of time.
You can simply go for an ultrasound to look for any features suggestive of fatty liver.
I suggest you to repeat the liver function test(LFT) after two weeks to check whether the levels persists or not.
Hope I have answered your query.
Thanks

Doctor called and said ultrasound showed hepatic steatosis with inflammation. He referred me to a gastroenterologist. Do you know what my treatment plan may be?
Brief Answer:
Conservative management will be the choice.
Detailed Answer:
Hello,
In most of the cases conservative management aiming to prevent further damage will be the treatment of choice.
In many cases, lifestyle changes can help reverse fatty liver disease. The risk factors which can worsen the liver function must be stopped or avoided. In your case obesity is a major risk factors and extra weight should be lost by exercises and diet control.
Have a nutrient rich diet that’s low in excess calories, saturated fat, and trans fats. Alcohol use should be restricted strictly as it is a major risk factor for liver cirrhosis.
Certain liver supplements like udiliv can also be used by some physicians.
Thanks
