Question: Abd crampy pain sometimes followed by salvatation and wretching occor sporadically with no regard to what type of food I'm eating. I've had UGI & Colonoscopy which showed large hiatal hernia & diverticulosis which I've never had to be treated for. The pain in starts in stomach region & spreads to bowels and intensifies til pain is on scale of 8-10. It lasts usually 15 minutes then is gone. My stomach feels best when I don't eat. None of my organs are tender except the upper Intestinal tract during episodes.

Brief Answer:
Sub-acute intestinal obstruction needs to searched for

Detailed Answer:
Hi.
Thanks for your query.
Noted the history and understood the concerns.
My thoughts after reading your history.
Large Hiatal hernia can cause the type of pain that you have mentioned. Associated GERD is instigated by any type of food causing reactionary salivation and retching.
Another reasons can be sub-acute intestinal obstruction as the pain is gone after 15 minutes. The cramps explain the same.
When the stomach and proximal intestines are free meaning when there is no food there is no reason for the cramps to occur hence you feel better when you are on empty stomach.
Diverticulitis cause pain in lower abdomen and is not cramp-like, it is sort of constant hence diverticulitis that is inflammation may not be the cause of your pain.
Such type of pain may not be identified by Upper GI Endoscopy or colonoscopy.
You needs to undergo contrast enhanced CT scan of abdomen with CT Enetroclysis which may show the cause and gives plan of management. Please discuss this with your Doctor and request for it.
Low RBC count and Iron needs proper evaluation by a Hematologist and may be the cause of breathlessness as there is no proper oxygenation.
I hop[e thyroid functions are within normal limits.

I hope this answer helps you to get a proper diagnosis and management.
