Question: Acute Hepatitis B reactive since 3 months
Blue ribbon is directly increasing
SGPT level is going down
Alkaline is 280
Whole body is itching because of which I am sleep deprived, and having wounds because of continous itching ,whole body%E2%80%99s color has changed to black.
here are some of the reports please give me a good advice.

I have reviewed your reports and below is my advise.

Your ultrasonography report doesn't indicate any specific Abnormality.

But as you have acute hepatitis B it can be the causative factor for your high bilirubin level.
There is presence of conjugated hyperbilirubinemia due to that and so itching present.

You need to take rest and avoid strenous activities.

For itching you can be prescribed ursodeoxycholic acid tablet or colestyramine tablet. It will help in relieving itching.

You need to take sugar cane juice, Apple juice, coconut water etc more to improve jaundice early.

Avoid high fatty foods.

I strongly suggest you consult nearby gastroenterologist and discuss all these.

my skin color is also changing, ive lost a lot of weight, and having regular fever. because of itching im having wounds on the body. Ive also consulted the gastroentrologist earlier but they also couldnt justify the proper disease. the medicines that you suggested are being taken by me already but there has not been any improvement for 3 months. im also a general surgeon Please suggest me a treatment or any physician to counsult anywhere around the world because the nearest ones are unable to discover of what actually is the disease.

I have reviewed your query and below is my further advise.

Your conjugated bilirubin is very high and your liver enzymes values also twice times elevated.

So acute hepatitis infection is still present.

You need to investigate further with...
_Prothrombin time estimation with INR
_serum total protein estimation
_HBV DNA viral load
_Hbeag testing by Elisa
_anti hbcab Ig M by Elisa

If your viral load is high and hbeag positive than you might need antiviral medication.

You need to take more glucose containing diet, sugar cane juice, Apple juice etc foods that are beneficial in hepatitis.

You need to restrict fatty foods.

You should be given lactulose syrup as well to avoid constipation as constipation that can lead to encephalopathy development.

You need to repeat your USG abdomen report as well.

I suggest you consult nearby good corporate hospital like Fortis, Apollo or XXXXXXX like hospital
if you are not satisfied with your gastroenterologist.
