Age 85 Female No BP & Diabetes . Recently Recovered

Question: Age 85 Female No BP & diabetes. Recently recovered from acute kidney injury & then fell from a chair resulting in hip fracture. Went for the traditional bone setting procedure & the process is still going on. While on bed for sometime, she developed stage 1 bedsore adjacent to the excretion tract. We've been using ciprofloxacin IV betadine cream & antibiotic powder for tge last 15days for bedsore relief. Are these measures enough to get it healed completely coz it's not spreading to the other parts. But it's taking so much time due to her being totally in bed because of the hip fracture. plz suggest any other medication for faster healing of the bedsore.

I have gone through your query and can certainly understand your concern,

Bed sores demand proper care and hygiene along with the application of antibiotics. It is good that it is not spreading, but you need to see if it is getting deeper or is there any formation of slough over it. Slough is a yellowish debris formed over the wound. Antibiotic ointments when applied over the slough is not able to act over the wound and therefore becomes useless.

Regular follow up with your treating surgeon is required, as he will be able to carefully remove the slough, which aids in easy and proper healing of the wound.

If possible, kindly take a clear and close up picture of the wound, so that I can comment upon more clearly.

Hello madam i have attached the images for your convenience. There has been no yellow debris right from the beginning, i.e from 20days, of the wound. Also we are in a remote area & we can not take her to hospital due to 2 reasons. 1) she can't walk because of the hip fracture. 2) The nearby town us around 15kms away. Plz suggest a better ointment other than Betadine along with a powder & necessary precautions to make the wound heal more quickly.

Thanks for writing back,

I have reviewed the attached pictures, and I can see that the wound is clean with no superficial infections.

You can apply Mupirocin ointment 2%, this is effective against any bacterial infections. You can apply this 3 times daily, after cleaning and dressing the wound.
