Question: Aloha. I've been experiencing pain in my goin area, right side for the past 2 years. I have no associated swelling of the lymph, bulges, etc. in the immediate area (my legs have been swollen for the same past 2 years). Doctors here don't know what it is and have been treating it as a muscle or tendon strain with Oxybutinen. I can't find anything online that helps me 'diagnose' the issue.

That being said, I would like to know a few more details of the condition, so that I can give you a list of most probable conditions.

1. If there is any swelling in the groin at all, how large is the swelling?

2. How did the pain begin?

3. What happens when you press the in that area?

4. Is the pain below or above the groin crease?

5. When do you get the pain in that area? Is it while walking, or stretching the thigh back, or only on pressing on the swelling?

6. Does the swelling change in size in different positions like sitting, laying down flat or standing?

7. Does the swelling in the legs reduce after getting rest overnight?

8. Have you got yourself tested for filariasis?

9. Please upload a few good quality images of the painful region from different angles and also of the swollen parts of your legs.

10. Do you have any other symptoms at all, like frequent fevers, weight loss, cough, night sweats etc?

1. No swelling in the groin area
2. Pain began slowly in intensity. Now it's just there constantly and I get relief from Oxybutinin and Tylenol for 3 or so hours until it becomes unbearable again. The pain eases when I lift my knee or lean over my leg.
3. Nothing, no pain.
4. On the crease, spreading below the crease.

7. Yes, slightly.
8. No. Didn't know what that was until I just googled it. The legs are swollen from the knees down (worse on the right leg) and there are patches of skin thickening near the ankles.

Pain without obvious swelling could be bony in origin.

Detailed Answer:
Hello and welcome back.

I was waiting to see if you upload any pictures so that I get a better picture of what is going on.

Based on your description and your follow-up answers, I suspect some bony or ligamentous injury or an inflamed bursa.

Such prolonged pain without any obvious localised swelling indicates a deep seated issue, and the head of femur is located deep to the muscles in the groin, and there are bursae present in that location to minimise friction, it is possible that the pain is related to one of these structures or a nerve entrapment.

You must consult an orthopedic surgeon and get yourself examined thoroughly to find out the cause.

An MRI scan of the region may aid in diagnosis, you must also rule out osteoarthritis of the hip joint.

The lower leg swelling maybe due to kidney injury or heart failure or low albumin levels in the blood, that needs to be looked into as well.

It is quite hard to figure out what is happening without the help of direct physical examination, I recommend you to consult an orthopedic surgeon and see how things progress and you can keep me in the loop so that I can do my bit.
