Are CRP And Caalpro Indicative Of Body Inflammation?
Question: Hello Doc,
Can you please evaluate the attached reports and help me understand the reason why the levels are raised of C - reactive protein.
1. We had done C - reactive and calportentin test in XXXXXXX 2018 the c - reactive was negative and calprotectin was very high so doc had prescribed one tablet and then after 4 weeks she did the calprotectin the reports were normal.
Now recently yesterday we did the C- reactuve test and the results are high. Can you please help understand why the C - reactive tests is so high .
My mom has been suffering from frequent motions from past 1.5 years and endoscopy was done in 2016 I am attaching the biopsy report as well. Please help understand and what other investigation we should do and what is the significance of C - reactive protein . Also i am attaching the urine report which says pus cells are present is that urinary track infection ?

I am sorry but there are no attachments with the question so you can attach reports with follow up.
Both CRP and Caalpro are an indicator of inflammation in the body.
However, CRP is just suggestive of inflammation in the body that could be anywhere from the stomach to the lungs.
In today's world CRP is an old school test and is usually helpful only in cases associated with arthritis and tuberculosis (clinically).
As per as calprotectin is concerned it's just a test suggestive of inflammation in intestines.
Its a test with low accuracy and colonoscopy has to be done to rule out carcinoma or inflammatory bowel diseases in such patients.
They are not sure shot tests like colonoscopy etc. They have low accuracy and are not very sensitive or specific.
For eg calprotectin can be increased even in cases as small as gastroenteritis (as it is also inflammation) and on the other hand in Inflammatory bowel disease (which is also inflammation).
As she is having UTI and pus cell there is inflammation in her urogenital tract so CRP, in general, is increased.

Presence of pus cells is always suggestive of infection in the urinary tract.
A course of antibiotic cefixime 200mg twice would be given.
Her calprotectin was increased because of ulcer in the terminal ileum.
Report clear cut says that terminal part of ileum. Had ulcer in the past.
Now, it might have healed itself, therefore, calprotectin is normal now.
Rx
Tab cefixime 200 twice daily for 5 days. It's a prescription medicine. Her colonoscopy revealed nothing else.
So I don't feel we need to be obsessed about CRP like old school investigation.
