Are ELISA And PCR Tests Conclusive After 17 Days Of Exposure?

Question: Dear Doctor, I had a sex with condom hardly for 15 minits 17 days back, but i sucked her breast milk XXXXXXX 10ml. I undergone the following test now : a) HBsAg Elisa, b) VDRL/RPR, c) HIV I & II ELISA (4th Generation EIA test), d)PCR. All these reports has come negative. Please let me know am I still at risk and should I undergo again for HIV test after 3 months? Please suggest me.

Please find my observations below.

1. Since you had sex with condom therefore risk of STDs and HIV is less likely. However testing is recommended appropriately even if the risk is small if you had sex with partner whose sexual health is not known.

2. HBsAg is the surface antigen in hepatitis B virus. It should be tested again at 6 months to confirm absence of infection.

3. VDRL RPR is to be tested after 30 days from incident to get accurate result.

4. HIV I and II antibody test is most accurate after 6 months. If your HIV I and II antibody test is negative after 6 months then it is accurate. p24 testing (usually comes as a combination test with HIV I and II) is very sensitive for HIV I infection but in XXXXXXX HIV II is also less frequently seen and therefore it is best for you to repeat at 6 months. PCR is not required to be repeated.

5. In conclusion, it is early to tell that you have no HbsAg or HIV infection. Please follow above guidelines and do repeat test at 6 months for accuracy of HbsAg and HIV report. The HIV I and II ELISA 4 generation rapid test is the latest available and is highly accurate for HIV I at 1 month but since risk of HIV II persists in XXXXXXX therefore I recommend doing the test at 6 months and by that time the result will be accurate for HIV I and II.

1. The PCR test is for children born to mothers having HIV. It amplifies the viral elements and can give an approximation of viral load. It is not routinely recommended as a screening test for adults post exposure.

2. Any fluid including breast milk might contain the virus in an infected woman.

3. Studies have shown and I quote that There is evidence to suggest that HIV infected CD4 cells have a greater capacity to replicate themselves in breast milk than in blood. However 10 ml milk is a small amount and risk is less.

Dear Doctor,
I have test again as Non-Reactive, attached the latest report with file name "Report 13.7.2015". I went for retest because it is exactly 4 weeks of incidence and you also have replied above as "The HIV I and II ELISA 4 generation rapid test is the latest available and is highly accurate for HIV I at 1 month".
Please let me know your feedback.

1. Your report mentions ELISA for HIV I and II non reactive. This means there is no HIV I and II antibodies at the moment as seen from the report.

2. My earlier reply about 4th generation ELISA test for HIV I accurate detection at one month includes p24 antigen and the p24 antigen is the one which is most accurate at one month. Since p24 antigen is not mentioned in the report, therefore I am confused if you have done 4th generation HIV test or older test.

3. Please do not worry as there is very less risk for HIV infection in your case but since you asked for validity of the test, I want to reconfirm if the test is 4th generation and if yes then why p24 is not mentioned in the report.

BTW, its 4th generation elisa. The 2nd test are also one in the same hospital and paid the same fees for. I need to cross check with them. What if the test is not 4th generation elisa test?

1. There is a lot of confusion in the commercial names of the tests, however these tests are available as test kits and the details are clearly mentioned and the laboratory will have details which can be confirmed.

2. The test which is confirmatory for HIV I at one month is called HIV-1/2 Ag/Ab p24 Combo test. It looks for HIV I and II antibodies and p24 antigen. This p24 is present usually in HIV I virus and if there is no p24 after one month then HIV infection is unlikely.

3. If the new test including p24 is not available then please do the ELISA for HIV I and II antibodies test (the test which you got done twice) at 6 months and then it can be called confirmatory for absence of both HIV I and II infection. You might also go to a good laboratory and ask if they do test for HIV p24 and get tested now.
