Are HIV Tests Result At 39 Days Post Exposure Conclusive?

default
Posted on Tue, 4 Jul 2023
Question: I had an condom protected vaginal sex with an unknown CSW 39 days ago. On 39 Days, got tested and results were for HIV 1/2 antibody + p24 (CLIA) non reactive. RPR non reactive (carbon flocculation), TPHA negative, HBsAG surface antigen (CLIA), Hepatitis C non reactive (CLIA), Herpes IgG for both 1 and 2 non reactive (CLIA). Just before the test was taking levofloxacin and loratidine for some acute ear infection.

Does the above tests and waiting period are conclusive and can I move on with my normal family life?
I did not tested chlamydia and gonorrhea and does it pose a serious risk for sex with my wife?
Does my taking medicine like this will have any effect on result?
Please provide me your overall opinion in my situation.

Brief Answer:
Your screening tests are fine. Repeat HSV ELISA at 12 weeks

Detailed Answer:

Hello,

Protected vaginal sex is safe as far as risk of transmission of HIV, gonorrhea and chlamydia is concerned. Your screening test for HIV at day 39 is conclusive as it was done beyond the window period (2-4 weeks) for a 4th generation screening test.

There is no need to screen for gonorrhea and chlamydia. A negative test result for VDRL and TPHA together at 4 weeks is good enough to rule out syphilis. HCV is not transmitted sexually. HBsAg negativity is fine at this stage.

Your medications would not have interfered with test results. I would only want you to repeat an ELISA for HSV at 12 weeks for a conclusive result.

Therefore, all in all you can get along with your normal life.

Hope I have answered your query. Let me know if I can assist you further.

Dear Dr. Kakkar,

Actually was looking for your name to post initially. Why your name is not coming the list of XXXXXXX doctors for derma? Please check with the website.

Thank you for your response. My further add on question would be, till today its been 52 days and I did not have any syphillis lesion etc. But from your experience test at 39 days (5 and half week) for HSV is indication for waht?
After the act, checked the condom with water and it was intact, so is there any other STD that you may consider for referral for testing? My wife will be joining me and can I have normal sexual life....please advice.....

Brief Answer:
You seem to be okay.

Detailed Answer:

Hi,

ELISA for HSV (IgG) is considered conclusive if it is detected negative at 12-16 weeks after a risky sexual encounter as it might take longer for it to appear in the blood. Therefore, you may repeat HSV ELISA (IgG) at 12 weeks and if that is negative, it would be conclusive.

Till then you may have protected sex with your wife. In my view, your lab tests have adequately ruled out all other STDs including syphilis (negative, VDRL and TPHA together) and moreover, since yours was a protected sexual encounter, there is no need to worry.

Hope I have answered your query.
