Are HSV 2 ELISA Test And IgG Test Conducted 4 Weeks Post Exposure Conclusive?

default
Posted on Fri, 27 Nov 2015
Question: I have sex with a new partner, it was protected except she conducted an oral sex without condom. Around A few days to a week later, I have some red spots on my penis.
The red spots are not pain, not itchy and do not have fluid inside. They were quite flat / smooth. The red spots almost now disappear after 6 weeks. They do look like genital herpes, except I don't have any fluid or pain on those spots.
I do have psoriasis and have spots mainly on my trunk. Those new red spots also look like psoriasis on genital area, but I am not sure if psoriasis on genital area will go away after a few weeks.
I conducted a blood test on genital herpes after 4.5 weeks of the exposure. It was a HSV 2 XXXXXXX test / IgG test. The result was negative. I doubt it was because I am still in the window period, but the clinic said that their test is highly accurate after 3 weeks of exposure, so after 4.5 weeks is good. I also test for HIV and syphilis, it was negative also.
I want to know if those red spots genital herpes or psoriasis on genital area? Should I do a blood test or other test later again?

Thank you.
I have uploaded 2 photos which were taken 4 weeks after the exposure (3 weeks ago). Now the spots are almost gone. Appreciate if you could tell if they are genital herpes or psoriasis or other infection?

1) I want to ask the risk of getting HSV 1 genital herpes through oral sex? is it high?
2) What is the chance to get HSV 2 genital herpes through oral sex?
2) Since the red spots look like either genital herpes or psoriasis on genital area, how could I tell? My spots are not itchy, not pain. I don't recall fluid came out from the spots.
3) My blood test was for HSV 2 IgG, should I do a test for HSV 1? If yes, what test?

Thank you.

Brief Answer:
You may also test for IgG, Hsv type 1

Detailed Answer:
Hi.

Thank you. I have reviewed the Images. I can see dy scaly patches/ spots. This is certainly part of psoriasis specially considering that you have skin involvement too.
Genital herpes usually presents with fluid filled vesicles which soon rupture to form erosions (i.e breaks in skin). There is pain and burning sensation.
Oral sex can transmit both Hsv type 1 and 2.
The differnce between these being that oral sex transmits hsv- 1 more commonly because Hsv- 1 is more suited to oral mucosa as compared to hsv- 2 which is more suited to genital mucosa.
The risk of transmission of herpes virus through unprotected oral or vaginal sex with an infected partner, is less in the absence of a clinical epsiode of herpes and is about 10%. With a clinical episode the risk of transmission is higher i.e 25-50%. I assume you sexual partner did'nt had a clinical epsiode at that time.
Yes, you may also do a blood test for IgG, Hsv type 1 because it is the predominant hsv type that infects oral mucosa.

Thank you.

Could you tell me what are the symptoms of genital herpes (Type 1 & 2)? I read a lot online and get confused. Some mentions the symptoms so vague that it can match with any red spots.

1. Particularly on the red spots, what will the genital herpes spot look like and feel like? E.g. shapes, pain or not, smooth or rough etc...

2. A few days back, I got a sore throat and flu-like symptoms (not not new red spots appear), is it related?

In addition to the previous questions, I want to ask will genital herpes (type 1) move to other parts of the body, e.g. become oral herpes?

Thank you.

Brief Answer:
Your lesions are not those of genital herpes

Detailed Answer:
Hi.

Genita herpes usually presents as 'painful' erosions/ sores on genital mucosa rather than as asymptomatic red, patches. Herpes is not scaly. Herpes sores are wet.
Genital herpes remains localised and cannot spread on to involve oral mucosa. Both hsv type 1 and 2 can cause genital herpes however type 2 is a more common cause of genital sores. Genital herpes is usually acquired by sexual contact whereas oro-labial herpes by kissing ot oral sex.
Flu and sorethroat, particularly streptococcal sore throat, can be one of the reason for flare of psoriasis.
