- Thoracic outlet syndrome
- a cervical disc bulging, which would lead to radicular chest and arm pain
- an inflammatory disorder.
The nailbeds changes in color may be related low blood flow, which could be caused by Thoracic outlet syndrome or Raynaud Syndrome.
For this reason, I would recommend consulting with your attending physician for some other tests:
- a chest X ray study
- a cervical spine X ray study
- inflammation tests (PCR, sedimentation rate, complete blood count)
- a nerve conduction study of the upper limbs may be needed coupled with a CT scan of the upper chest.

Yes, gerd could cause such chest pain. Have you ever tried taking an antiacid? Has it relieved your symptoms?
Anxiety can also mimic all this clinical situation.

Regarding your thumb shooting pain they are definitely not related to the heart. A nerve disorder can be the possible cause (shooting pain is usually related to nerves or muscles). A nerve conduction study of the upper limbs could be helpful in this situation.
I would not recommend repeating thyroid hormone levels, as you have performed them within the last year.
Anyway, your symptoms do not seem to be related to any serious medical condition.
