Thanks for your query ,based on the facts that you have posted it appears that one of your 48 years old family member has Enlarged Testicle .

1) Does he have fever with chills
2) Since how long he has enlarged testicle ?
3) Does he have severe pain in testicle ?
The commonest cause of sudden enlargement of testicle is infection (Orchitis) where as long duration enlargement is due to cancer of the testicle
Consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Ultrasound Scanning of Scrotum.
Further treatment will depend upon result of these tests and final diagnosis.

in answer to your questions: The answers to the first and third questions are no. The second questions is that it came on fairly suddenly a couple of months ago and he noticed it’s gotten worse over just the last few days.
He needs detail evaluation .
