Brief Answer:
Do a urine test

Detailed Answer:
Hello ! Thank you for posting your question here.

The pain that you describe in your back could be a muscle pain ( from stretching ).When your pain start after you change position ,if it's not a constant pain , goes away after you relax in you bad goes for a muscle pain.
Please give me some more information so I can have a clear idea of the cause :
Did you have any history of trauma in that area that may cause that?
You have the pain only when you may down to sleep or also sometime during the day.?
When you put pressure on the area does the pain
change ?
For how long have you been with pain ?After or before blood in urine ?
Is the pain worse or better since it started ?
Do you have any other symptoms that you did not write ( burning during urination)?
Do you use any medication?

If you had trauma in that area your blood in urine could be related to that.

You should do a urine and blood test to check your kidney function ,for microscopic hematuria , kidney stones , kidney infection, check your blood pressure also.
Use warm compreses or a topical antiimflamator for the back pain.

Take Care !
Dr Balliu.
