Question: My 2 1/2 year old son has sores on the bottom of his tongue. Seems like red irritated tissue inside mouth. Possibly low fever.

Hi,

Mouth Ulcer can be Aphthous Ulcer or sign of early hand foot mouth disease.

In aphthous Ulcer, there is Ulcer in the oral cavity which improves within a few days and the underlying cause is not very well known.

In Hand foot mouth disease the oral Ulcer can be the initial symptom with fever and later on there can be rashes on the hand and feet.

In both the condition application of the gel containing choline salicylate which is an analgesic and anti Inflammatory with lignocaine an anesthetic agent over the ulcers helps in relieving pain and redness.

It can be applied 3-4 times a day. Multivitamin supplements help in the early healing of ulcers. I hope with this treatment the ulcers will improve within 4-6 days.

If fever is more than 99 degree you can give paracetamol to bring down the temperature.
In summary, I would suggest you apply the gel (as said), to give multivitamins, paracetamol for fever and observe for other symptoms.

If you find any significant problem please consult your pediatrician immediately.

Appears to be allergic or early viral rashes

Detailed Answer:

Hi,

Itchy red rashes of different sizes and morphology suggests more of allergic etiology. As there are associated mouth ulcers and low grade fever, these rashes can be early stage of viral infection also.

The management is same in both the conditions. Apply mouth gel as advised. Giving antiallergic like cetirizine or hydroxyzine will help in rashes. Application of soothing agent like calamine / caladryl lotion will also help.

Observe for any improvement or worsening on condition like high grade fever, rapid progression of rashes, formation of ulcers, any breathing difficulty or any other significant symptoms and if you notice any please consult your pediatrician immediately.
