Question:
The tests were done to investigate low sex drive in a male 54 years old who lives a high stress lifestyle with poor sleep quality. I am attaching the blood test results and your initial response below.
Male 54. Hormonal panel results: SHGB 35nmol/L; range 10-50 Testostwrone 350ng/dL; range 250-827 FSH 2.8mIU/ml; range 1.4-12.8 LH 1.2mIU/L range 1.5-9.3 Also Vitamin D 28.5ng/mL; range >29.9 Are these results worrisome? I’m worried whether a serious condition could be lowering the LH. Thank you.
Doctor's Chandrakant's Answer
LH in males help to make testosterone and sperms. Your testosterone is normal. May need supplements of Vit D as it is just below normal. What are your symptoms? Any problems? Please give details. Usually hormone panel in males is done not as a routine. ..

Brief Answer:
stress and lack of physical and mental rest are major causes.

Detailed Answer:
Hi.
Thanks for your query.
Noted the history and findings.
HSGB that is sex hormone binding globulin is 35 (10 to 50) means normal.
Testosterone is 350 (250 to 827) that is within normal limits.
FSH is normal 2.8 (1.4 to 12.8)
LH is luteinizing hormone : 1.2 (1.5 to 9.3) just below the lower of normal range. It helps in production of testosterone, which is within normal range as per your reports. It is just below normal. Not too low.
Vitamin D is 28.5 and just below lower range of normal. Needs supplements.
History of taking Atorvastatin for so long - 10 years worries me. High cholesterol has many causes or mostly a combination of multiple causes like sedentary life, too much oily foods, thyroid problems, dyslipidemia, alcohol, obesity and so on.
The causes for low sexual drive can be combination of many factors.
Anxiety, stress and related problems is one of the commonest cause.
Poor sleep is another cause.
Low testosterone (yours is normal)
Many medicines cause low sex drive like those for blood pressure, even statin like atorvastatin
Diabetes
Thyroid ; low hormone levels of T3 and T4
I hope you have undergone tests for diabetes, liver, kidney and thyroid function, lipid profile. If yes then please post reports. if not done then get these done.
Please give additional information:
Since when is the low sex drive present?
What exactly is the problems? Low drive/ erectile dysfunction/ not sustaining the erection or early ejaculation? Please give exact things you are suffering from?
Any discord with sexual partner?
What is your weight and height = BMI?
Any habits like smoking, alcohol or any addiction?
Anything else that you would like to share with me.
Awaiting for your feedback.

Hello and thanks for your response.

Weight is 100kg, height is 182 cm, so I am indeed overweight.

I do exercise with weights but not as much as I would like to, because of my schedule and because I feel tired in the evenings after work. I have started walking and will try to get more sleep. I will also try to use weights and train more frequently.

No smoking. I used to be a social drinker in the past but do not drink at all any more and have not had alcohol in over two years.

I have had metabolic tests and urinalysis as well as thyroid panel and TSH. The urinalysis tests were all normal. CBC tests were all normal. My HDL cholesterol is low.

Brief Answer:
Reduce obesity , exercises daily, change routine,

Detailed Answer:
Thanks for your feedback.
Your BMI is 30.2 that comes under obesity, which itself is the reason for many problems as I discussed earlier. Correction of obesity will improve everything for sure.
Cholesterol is 141 and is perfectly normal. Ask your Doctor if you really need Atorvastatin. If he advises and if you reduce your BMI to normal you will not need such medication which can be cause of sexual problems.
Exercise needs to be a daily affair, brisk walking or cycling are the best in your case.
Stop Gluten for a month and this may improve many of your problems. If it shows positive results stop gluten for a long time/lifelong. Wheat and other high gluten products - list is available online.
Erectile dysfunction - just over 6 months - the commonest cause is stress, anxiety, weakness, low energy by the evening. Regular exercises boosts all positive things in life.
You may need to take a break from routine. One does not loose anything if you so it for improving your life which is the most important part of life.
I hope this answer helps you.
